Your Request for Live Scan Service form issued to you by your employer or referring agency will have a ORI CODE. The ORI CODE will determine the level(s) of Live Scan Service you will require for your license or position. Please call us at (818) 892-6141 if you have questions!
In order to properly prepare for your Live Scan appointment, you must present a valid photo ID to the Live Scan technician. You will need to bring ONE of following DOJ approved forms of photo identification:
- U.S. Passport
- California Driver’s License
- California DMV ID Card
- Out-of-state Driver’s License
- Alien Registration / Immigration Green Card
- Military ID Card
- Mexican Consulate ID Card (Matricula Consular de Alta Seguridad)
A temporary license (paper printout from DMV) is not a valid form of identification. Please complete the Request for Live Scan Service Form prior to your appointment and bring your Social Security Card. Expired photo identifications will notbe accepted
Your Live Scan fee is the service provider “Rolling Fee” plus any government processing fees that are called for on your “Request for Live Scan Service” form. Note: If your Live Scan form has a billing number in the “Agency Billing Number” line (this is a six digit number that starts with the number “1”), that means that the agency requesting your Live Scan will be billed for the background fees and all you will need to pay is the $23.00 fingerprint rolling fee. If your form does not have a billing number on it, you will be responsible for paying both the rolling (fingerprinting) fee and the background fees.
Re-submission: If for any reason your fingerprints need to be resubmitted, DOJ and FBI fees will not apply. We do not charge any rolling fee for resubmission if our agency processed your initial fingerprints.
